School Fees: School fees for the 2023-2024 school year are posted and are due by October 30, 2023. Past year's fees and outstanding CLSD property are past due. Any student who participates in co-curricular or extracurricular activities that require school transportation must have all school fees paid and missing items returned. The release of records and diplomas will be held until all debts have been resolved. A payment plan for instructional fees may be arranged by contacting the office with full payment to be received by May 15 of the current school year. If you feel your children may qualify for free or reduced price meals, please complete the application on the PaySchools website. Payment options: Pay through the PaySchools system on the school's website, mail payment with check made payable to Columbia Middle School, or send check or exact cash with your student to pay in the office. If you have any questions, please contact the office at (440) 236-5741.
 
Back to School Forms: Forms are OVERDUE on your OneView account and must be completed immediately. Log in to the OneView site using your students six digit ID number. You must complete/review the EMF every school year. After completing/reviewing, please go to the bottom of the form and sign, date and submit. If you need assistance, please contact the office.
 
Picture Day is Tuesday, October 3!! Cavanaugh Photography will NOT have paper flyers available on picture day. Click HERE for the flyer. You may print it out if you would like to pay with cash or check. Otherwise, follow this LINK to order online. Online ordering is now available and will be available for 48 hours after picture day. 
 
Parent Teacher Conferences: Conferences will be held on Wednesday, October 11 and Thursday, October 12. All conferences are scheduled to be in-person this year. If you need a virtual conference, please reach out to the team of teachers to set up a conference. Once again, we are using PTCFast.com to make signups as convenient as possible. 7/8 grade Wrestling: Sign ups are in the ofice for any 7/8 grade student interested in wrestling this year. See Mr. Juliani with any questions.
 
Join us at the 7th Annual Fall Family Math and Literacy Night on the Farm! We will tie in fun activities to reading and math while you enjoy your time outdoors at this popular fall event! The event will be held on Tuesday, October 17, from 4:30 until dusk at Red Wagon Farm. The cost is $5 per person. Hope to see you there! Chromebook Insurance: Chromebook Insurance is available for the 2023-2024 school year. Indicate your choice to purchase insurance on your OneView form. Cash or check payable to CMS may be dropped off in the office. This optional insurance must be purchased by October 6, after that date it will not be avaiable. Insurance may not be paid on the PaySchools site. See chromebook letter HERE

Supply Lists: Take advantage of summer sales and pick up your 2023 fall school supplies early (toggle on left).
 
Report Cards: You may access and print a pdf. of your student’s report cards in PowerSchool. Please note, report cards cannot be viewed on the app. You must login to the PowerSchool website to view and print report cards. Access PowerSchool from the MS website (toggle on left).
 
Incoming 7th grade students: The Ohio Revised Code 3313.67 requires that a student entering 7th grade provide written evidence of being immunized against tetanus, diphtheria, pertussis, (Tdap) and meningococcal (MCV4) or is in the process of being immunized against these diseases.
You must provide the school with a current immunization record for your child by September 18, 2023.
 
School Fees: Fee letters were mailed in June to students with outstanding fees. Information on payment and due dates are listed on the fee letter. 2022-2023 school fees are overdue. Please pay to enable students to use school transportation for extracurricular activities. 2023-2024 Fees will be assessed at the end of September and sent home.
                      
Medications: All medications kept at school must be picked up at the end of each school year. Remaining medication not picked up by a parent will be disgarded. Please remember that a new medication form must be filled out each school year by your physician. Contact Nurse Mindi with any questions at [email protected]

Columbia Youth Football and Cheer Information: This league is designed for children entering K-6th grade in the 2023 school year and has the following levels to sign up for: K-2 grade Flag, 3-4 grade Padded Flag, 5-6 grade Tackle, & K-6 Cheer. Chromebooks, Chargers and Insurance:  Students are responsible for keeping chromebooks in good working order. If you have a damaged or chromebook that is not working properly, bring it to the office for repair.  If you have lost or damaged your charger, you are responsible for replacing it. They are available on Amazon. Make sure you check the model name before ordering a replacement. Chromebook Insurance will be available in fall 2023 for the 2023-2024 school year.

PowerSchool Parent Portal: Please note, report cards cannot be viewed on the app. You must login to the PowerSchool website to view and print report cards.

2023-2024 School Year: 
School Day: 7:30 am – 2:30 pm.
Buses arrive at the school by 7:20 am.
Car riders drop-off begins at 7:20 am. 
Car riders dismissed at 2:30 pm.
Bus riders dismissed at 2:35 pm.
 
School Fees: Check your child’s PaySchools Account (Pay ForIt) or PowerSchool account to view your students outstanding fees. 
Students who participate in extracurricular activities that require bus transportation will be denied that privilege if school fees are not paid. Fees may be viewed on your child's PowerSchool account and paid on PaySchools with a credit card. Payment is also accepted in the office with cash (exact amount), check or credit card or mail your payment to the middle school. If you require a payment plan, contact Mrs. Bowes in the main office. A signed payment plan contract will allow students with outstanding fees to continue to use school transportation.

Athletics

7th and 8th grade Student-Athletes: All 7th and 8th grade student-athletes must complete the athletic forms on OneView prior to the first mandatory practice of the season. A current OHSAA Physical Form must be uploaded in PDF format onto the OneView site and a hard copy must be submitted to your child’s coach no later than the first official day of practice. Students will not be permitted to participate in any practice without the submission of the online forms on the OneView portal on our website. The hard copy of your child’s current physical form must be on file or submitted to your child’s coach no later than the first official day of practice. See the Oneview instructions HERE for first time users to create an account. OHSAA sports physicals are valid for one calendar year. The 2023/2024 OHSAA Preparticipation Physical Form is available in the information rack across from the office or downloaded HERE  

7th and 8th grade Participation Fees: The 2023-2024 athletic participation fee is $150 per athlete per season with a $300 family cap per season for athletes in 7-12 grade. This fee is due no later than the first athletic contest. School transportation for all extra curricular activities will be denied until all past and current school fees are paid.
