High School Athletic Forms and Information
Athletic forms are available on the OneView link on our website. The 2023-2024 prepartcipation physical form is available for download. Once complete please upload to your child's OneView account or bring original to the main office and we will upload for you.
The physical is due the first day of mandatory practices. Remaining online forms should be completed by the first day of practice for your child. The physical form is valid for one calendar year and will be kept on file. The code of conduct, eligibility checklist, medical waiver, and the yearly mandatory preseason parent information need to be updated on your OneView account each athletic season to ensure CHS has the most current and accurate information to ensure your child's safety.
Your child's participation fee of $150 for the 2023-2024 school year is due no later than the team sports picture day scheduled for the season in which your child will participate. This fee is $150 per season (fall, winter, spring) per athlete; with a family cap of $300.00 per season for 7-12 athletes. The fee for Esports is $50 per season (fall and spring).

The OHSAA eligibility guideline is applicable for all school athletes. Please take the time to read the document carefully.
Prior to the opening of each season, a photo day is provided for individual and team pictures. Reminders will be sent home with all athletes. The current season photo schedule can be viewed on our high school home page under daily announcements. The mandatory preseason parent info is available on the High School OneView site.
The Columbia Athletic Booster Club works with the Athletic Director to ease the financial burden of extra-curricular athletic activities, grades 7th through 12th, on the school system and parents. All money raised goes back to student athletes in the form of equipment, uniforms, scholarships and more. Join us in support by becoming a member. Athletic Season Passes for the 2023-2024 seasons are available for purchase in the high school and middle school offices.
Columbia High School is proud to have an Athletic Hall of Fame with the first inductees honored in 2015. Nominate a deserving athlete, coach, school official or community member on the nomination form and return to the athletic office at CHS.
